Our initial assessment takes place during the first home visit upon contacting us. During this initial assessment, we take note of the client’s needs according to what we have identified at that time.
But, needs can change. So, we make sure to conduct continuous in-home assessments to address the ever-changing nature of every client’s needs. The results of these assessments will translate to changes in the care plan, eventually modifying the services that a client should receive thereon.
